  1. All covariates were entered in one block together with the respective distress measure. Relative risks are expressed for a 2-point increase on numeric rating scales for distress measures and for a 5-year increase for age (i.e., age values were divided by 5 before entering the equation). Because of missing data for hypertension and smoking status, all models included 286 patients and 43 cardiovascular disease-related events.
